Outpatient Records


Airman First Class Ann N. Rocha distinguished herself by meritorious service as Patient Administration Technician, 18th Medical Support Squadron, 18th Medical Group, Kadena Air Base, Japan. During this period, Airman First Class Rocha managed the largest outpatient records section in PACAF and maintained more than 17,000 medical records, ensuring the availability of those records and enabling over 6,300 medical appointments monthly. Further, she was instrumental in tracking, researching, and mailing out medical record requests for civilian, military, and veterans administration treatment facilities, facilitating seamless and effective medical care for 35,000 beneficiaries. Additionally, she completed the annual medical records retirement and active duty inventory, accounting for 12,000 active duty records and archiving over nine hundred with zero errors. Finally, she served as the Outpatient Records section NCOIC for over three weeks, managing five members, and servicing over one thousand patients, reducing the copy request workload by more than thirty percent. The distinctive accomplishments of Airman Rocha reflect credit upon herself, the 18th Medical Support Squadron and the United States Air Force.




Senior Airman Shawn M. Lewis distinguished himself by outstanding achievement while serving as Outpatient Records Lead, 377th Medical Group, Kirtland Air Force Base, New Mexico, from 12 April 2012 to 14 October 2012. During this period, the professional skill, leadership, and ceaseless efforts of Airman Lewis contributed to the effectiveness and success of Air Force programs. In addition, Airman Lewis assisted an Air Force wounded warrior who had suffered a traumatic brain injury in locating his medical records that were needed to support medical claims but which had been misplaced for several months. The medical records were found and validated, enabling a medical evaluation board to resume the determination of Line of Duty and medical resolution of benefits for the wounded warrior. The distinctive accomplishments of Airman Lewis reflect credit upon himself, the 377th Medical Group, and the United States Air Force.
